I am writing to inform you that due to some change in my personal circumstances, I will no longer be practicing at Pioneer Valley Cardiology Associates (PVCA) in 2016. I have been truly blessed by having wonderful patients and I am deeply grateful for your trust during our time together.
Please be assured that despite my leaving, you will continue to receive excellent care by any of the physicians in the practice. All PVCA physicians and staff stand ready to continue your cardiac care. We also have an excellent panel of physician assistants (PAs) and nurse practitioners (NPs) who are available to provide care to you.
If you are scheduled for follow-up care, your appointment will be re-directed to another provider. If you are due for a follow-up visit in the coming months, this has been noted in our appointment recall system. You will receive notification from our office to schedule an appointment. Our staff is prepared to help all patients make an appointment with one of our current physicians, a new physician hire, one or our NPs or PAs or another physician outside our practice. We have excellent friendly staff that is ready to help.
I am also happy to tell you that PVCA has hired two new physicians who will begin work this summer. Christopher Russo, MD will be joining the practice in August, 2016 after completion of his Fellowship at Dartmouth Hitchcock Medical Center in Lebanon, NH. Cheri McGregor, MD will also be joining the Practice this August. Dr. McGregor is completing a four year commitment at the Travis Air Force Base, after completion of her Cardiology fellowship in 2011. For patients who were not scheduled for follow-up until August or beyond, I am confident that you will find Drs. Russo and McGregor available and more than capable of taking care of your cardiac needs.
On a personal note, my years with Pioneer Valley Cardiology Associates have been very rewarding. I leave confident that my colleagues will continue the tradition of excellence and caring that have guided me through my years of practice. You will continue to be well served.
With sincere thanks for our past relationship, and warmest wishes for the future, I remain.
Zareen Farukhi, MD.

It has been a privilege and a pleasure to be involved in your care over the time we have known each other. I am writing to notify you and your family that I will be retiring from Pioneer Valley Cardiology Associates (PVCA) on September 25, 2015.
All physicians and staff of PVCA stand ready to continue your cardiac care. I can recommend all of my cardiologist partners enthusiastically and without reservation. While our efforts to recruit new physicians to our Practice have not yet resulted in new hire(s), we anticipate this will happen in the near future. In the meantime, if you are scheduled for follow-up care in the coming year, this will be noted in our appointment recall system. Once we have hired new physicians, their arrival will be announced. In the interim, my physician partners and our team of Nurse Practitioners and Physician Assistants will provide care to you as needed.
Our office staff is prepared to help all patients who wish to make a transition to one of our current physicians, a new hire or another physician outside our practice. We have an excellent and friendly office staff that is ready to help.
On a more personal note, I have been fortunate and grateful to have worked with so many wonderful patients. I thank you for the very special gift of your trust. My years with Pioneer Valley Cardiology Associates have been very rewarding. I leave confident that my colleagues will continue the tradition of excellence and caring that have guided me through my years of practice. You will continue to be well served.
With sincere thanks for the past and warm wishes for the future.
Robert V. Chircop, MD
